Urine testing (urinalysis, pregnancy)

Skin procedures (abscess drainage, mole removal, skin tag removal)*
Suture placement & removal

*Pathology fees apply for Pap smears and biopsy specimens per Ohio law.
Monthly Membership fee:
Individual: $125
Family of 2: $240
Family of 3: $325
Family of 4: $400
Additional family members can be added for $50 per month.
Initial Visit fee:
$150 per member
Includes first month of care
Max $475 per household
Home visits are available at the discretion of Dr. J for a flat fee of $100.
Memberships may be paid monthly or annually. There is a 3% service fee if membership fee is paid for with a credit card. There is no contractual membership length of time. You can cancel at any time without penalty. There is a $225 fee to re-join the practice.
A limited number of discounted memberships are available for patients who need financial assistance. Please reach out to Dr. J directly for details.
Osteopathic manipulation treatment (OMT) is a hands-on technique used by physicians to address structural problems in the body. OMT can help alleviate pain and enhance mobility. The assessment involves feeling muscles and joints to diagnose tissue changes. Various treatments techniques are used to restore structural integrity. Dr. J uses gentle osteopathic techniques.

For non-members:
Initial treatment: $250
Follow-up treatments: $175
There is a 3% service fee if paying with a credit card. This fee does not apply if paying with ACH transfer.
