Home visits are available at the discretion of Dr. Jergovich for a flat fee of $100.
Initial Visit Fee:
$125 per member
(max $450 per family)
The initial visit fee covers the review of medical records and a 90-minute new patient appointment. It is billed with the first monthly fee when you join the practice.
Monthly Membership fee:
Individual: $125
Family of 2: $240
Family of 3: $325
Family of 4: $400
Each additional family member $50 per month.
Memberships may be paid monthly or annually by ACH transfer or card. The membership fee is HSA and FSA eligible. There is a 2.99% service fee if the membership fee is paid for with a credit card.
There is no minimum membership length. You can cancel at any time without penalty. To rejoin after cancelling, you will need to sign up on the waitlist. There is a $225 fee to re-enroll in the practice.
